af0f0300f8 PDP11, Unibus VAX: DEC standard 144 corrections and RP device update
- Added a specific drive type RP05 which is the same size as the RP04
  but can be distringuished by OS software.
- Restrict SET rpn BADBLOCK to only disk types which actually supported
  the DEC Standard 144 bad block table.
- Fixed typo's in help and comments about DEC 044 vs DEC 144 and also
  in pdp11_doc and vax780_doc documentation files.

a015e44c10 ALL: Standardize writelock behavior adding global UNIT_WLK & UNIT_WPRT
Historically this functionality was reimplemented within each
DEVICE simulator often with slightly different implementations
and inconsistencies.  Solving this globally within SCP required
changes in many places, but should henceforth be reasonably
managed.

- Generalize the attach help to be specific to the device/system being

5531ccb175 ALL: Massive 'const' cleanup
These changes facilitate more robust parameter type checking and helps
to identify unexpected coding errors.

Most simulators can now also be compiled with a C++ compiler without
warnings.

Additionally, these changes have also been configured to facilitate easier
backporting of simulator and device simulation modules to run under the
simh v3.9+ SCP framework.

1af590d806 PDP11, VAX8600, VAX780, VAX750: Fix to auto configure Massbus adapters
When a mix of Massbus devices are configured with some enabled and
others disabled, the MBA's need to be allocated and properly configured
in the desired preferred order (RP, TU, RS).  On the PDP11, this interacts
with auto-configure since the RH devices are visible in the Unibus I/O
page.  On the PDP11 the second Massbus device can only be configured
if the TM device is disabled since the auto-configure assigned vectors
overlap for RHB and TM.

Problem originally reported in #301.

f0d41f15d7 PDP11: Fix to correctly set PS value on CPU reset to reflect the model specific ways real hardware behaved. (from Bob Supnik)
Here's a PDP11 SIMH bug as old as the simulator itself: the reset_cpu routine sets the PS to 340 (interrupts disabled). This causes some versions of Lunar Lander not to work. In fact, the initial state of the PS is not architecturally standardized:

      04: cleared (from schematics)
      05: cleared (from manual)
      20: cleared (from schematics)
      34: cleared (from schematics), set to 340 on boot?
      40: cleared (from schematics)
      44: cleared on init, set to 340 on boot (from schematics, manual)
      45: cleared (from schematics)
      60: cleared (from schematics)
      70: cleared (from schematics)
      T11: set to 340 (from spec)
      LSI11, F11: 4 mode behavior (from memory on power recovery, cleared on GO, 340 on boot, mode 3 undefined)
      J11: 4 mode behavior (from memory on power recovery, cleared on GO, 340 on boot, 340 on jump to  custom PROM)

The story seems to be this. All non-VLSI PDP11s used TTL chips to implement the PS, either discrete flip-flops, or 4b registers, or both.
Starting with the first system, the 11/20, they were wired clear on the processor INIT signal (power-up or front panel START switch), so that all internal state started as 0. This worked fine, because START also reset the Unibus and cleared all interrupt enables. So even though the processor was as IPL = 0, no interrupts were possible. Then along came the LSI11...

The LSI11 implemented a line-time clock with NO INTERRUPT DISABLE. Thus, if IPL was left at 0 and a bootstrap routine from a slow device was started (e.g., a floppy drive), the clock would tick, and an interrupt would occur, before the bootstrap routine finished. Because no vectors were set up, the processor would crash. So the LSI11 started the practice, carried over to all later PDP11 VLSI chips, of setting the PS to 340 before jumping to a boot ROM.

The T11 did this in all modes of startup, because its only startup behavior was to jump to a "boot" routine. It did not have a console of any kind.

Accordingly, it appears that the cpu_reset routine needs to set the PS based on the processor model. Further, all boot routines need to set the PS to 0 or 340 based on the processor model. (It's probably safe for boot routines just to set the PS to 340, but it's not technically
accurate.)

1141339e5b PDP11: Addition of the RS03/RS04 disk devices from Bob Supnik
Note that this is not the fully general Massbus implementation people were dreaming of. MB#1 is for RP/RM, MB#2 is for TU, and MB#3 is for RS.
I know that there were requests for four Massbus channels and multiple RP/RP subcontrollers, but if people want more disks, they can use the RQ.

659873bebb The Massbus byte count register was incorrectly presumed to be constant during a complete I/O transfer request (it can change due to DMA activity). Prior to the conversion to use the sim_disk library, the Massbus byte count was only read once. After the code restructuring it was read early in the processing and again later in transfer processing. The subsequent read returned a different value which ultimately caused the problems. 13fd6e5cb7 Fixed a few minor issues:
1) a RELEASE is documented (in our limited case) to be equivalent to a CLEAR not a NOP.
   2) avoid setting Attention as a consequence of processing an UNLOAD command.
   3) extended debugging information needed while looking for an issue which turned out to be a bug in the vax 780 Massbus interrupt triggering logic.

87c3e3452f Added Asynch I/O and Disk Support for various Disk formats
I’ve always wanted to have the option to have simulated devices behave
more naturally with respect to I/O operations.  By more naturally I
mean that the current simulator model I/O is either polled (for asynchronous
things link Muxes and Network), or it is performed in the middle of some
instruction execution taking possibly many milliseconds (disk and/or tapes).
The existing model creates quite deterministic behavior which helps to debug
and understand issues, but it trades off potential instruction execution
while performing these I/O operations in between instruction execution.

To address this concept (while still retaining the potential advantages of
the original model), I’ve designed an Asynch I/O model extension for simh.
In order to flesh-out and debug this design, I’ve also refactored several
devices to utilize this capability.  Please read the attached
0readmeAsynchIO.txt file for concept details about the approach.

In order to make disk devices easy to implement (within or without the
AsynchIO framework), I’ve created a sim_disk.c  library which is modeled
on the sim_tape.c library to generalize disk I/O like tape I/O is
generalized in sim_tape.c.  This sim_disk.c library now provides that
natural place to implement support for various disk implementation formats
(just like sim_tape support several formats, and one day will be the place
to add direct physical tape access). The current sim_disk library provides
the framework for direct support of 3 different disk formats:
    1) standard simh disk format
    2) platform specific physical disk access
and 3) platform independent Virtual Disk format.
The Virtual Disk format is an implementation of the format described in
the ”Microsoft Virtual Hard Disk (VHD) Image Format Specification”.  The
VHD specification is available for anyone to implement under the "Microsoft
Open Specification Promise" described at
http://www.microsoft.com/interop/osp/default.mspx.
The VHD implementation includes support for:
    1) Fixed sized disks
    2) Dynamically expanding disks
and 3) Differencing Disks.
Dynamically expanding disks don’t change their “Virtual Size”, but they
don’t consume disk space on the containing storage until the virtual
sectors in the disk are actually written to (i.e. an RA81 or RA92 VHD
with a VMS installed on it may initially only contain 30+ MB of files,
and the resulting VHD will be 30+ MB).  The VHD format contains meta data
which describes the virtual device.  Amongst this meta data is the simh
device type which the VHD was originally created as.  This metadata is
therefore available whenever that VHD is attached to an emulated disk
device in the future so the device type & size can be automatically be
configured.

Sim_disk_attach is used by device emulations to attach a simh/vhd/raw
device to a simulated device.  The following simh command switches
are used by the sim_disk_attach API:

    -R          Attach Read Only.
    -E          Must Exist (if not specified an attempt to create the
                indicated virtual disk will be attempted).
    -F          Open the indicated disk container in a specific format
                (default is to autodetect VHD defaulting to simh if the
                indicated container is not a VHD).
    -X          When creating a VHD, create a fixed sized VHD (vs a
                Dynamically expanding one).
    -C          Create a VHD and copy its contents from another disk
                (simh, VHD, or RAW format).
    -D          Create a Differencing VHD (relative to an already
                existing VHD disk)

1.3.1 All ASCII terminals

- Most ASCII terminal emulators have supported 7-bit and 8-bit
  operation; where required, they have also supported an upper-
  case only or KSR-emulation mode.  This release adds a new mode,
  7P, for 7-bit printing characters.  In 7P mode, non-printing
  characters in the range 0-31 (decimal), and 127 (decimal), are
  automatically suppressed.  This prevents printing of fill
  characters under Windows.

  The printable character set for ASCII code values 0-31 can be
  changed with the SET CONSOLE PCHAR command.  Code value 127
  (DELETE) is always suppressed.

1.3.2 VAX-11/780

- First release.  The VAX-11/780 has successfully run VMS V7.2.  The
  commercial instructions and compatability mode have not been
  extensively tested.  The Ethernet controller is not working yet
  and is disabled.

3.1 SCP and Libraries

- The VT emulation package has been replaced by the capability
  to remote the console to a Telnet session.  Telnet clients
  typically have more complete and robust VT100 emulation.
- Simulated devices may now have statically allocated buffers,
  in addition to dynamically allocated buffers or disk-based
  data stores.
- The DO command now takes substitutable arguments (max 9).
  In command files, %n represents substitutable argument n.
- The initial command line is now interpreted as the command
  name and substitutable arguments for a DO command.  This is
  backward compatible to prior versions.
- The initial command line parses switches.  -Q is interpreted
  as quiet mode; informational messages are suppressed.
- The HELP command now takes an optional argument.  HELP <cmd>

  in the memory protect and DMS functions.  The moving head disks
  (DP, DQ) were revised to simulate the hardware more accurately.

- PDP-11/VAX Ethernet setting flag bits wrong for chained
  descriptors (found by Mark Pizzolato).

5. General Notes

WARNING: The build procedures have changed.  There is only one UNIX
makefile.  To compile without Ethernet support, simply type

	gmake {target|all}

To compile with Ethernet support, type

	gmake USE_NETWORK=1 {target|all}

The Mingw batch files require Mingw release 2 and invoke the Unix
makefile.  There are still separate batch files for compilation
with or without Ethernet support.

WARNING: V2.10 has reorganized and renamed some of the definition
files for the PDP-10, PDP-11, and VAX.  Be sure to delete all
previous source files before you unpack the Zip archive, or
unpack it into a new directory structure.

WARNING: V2.10 has a new, more comprehensive save file format.
Restoring save files from previous releases will cause 'invalid
register' errors and loss of CPU option flags, device enable/
disable flags, unit online/offline flags, and unit writelock
flags.

WARNING: If you are using Visual Studio .NET through the IDE,
be sure to turn off the /Wp64 flag in the project settings, or
dozens of spurious errors will be generated.

WARNING: Compiling Ethernet support under Windows requires
extra steps; see the Ethernet readme file.  Ethernet support is
currently available only for Windows, Linux, NetBSD, and OpenBSD.
