c5ce339566 AltairZ80: ADCS6: Fix uninitialized unit structure
Proper initialization of the ADCS6 unit data structure depended on the
ADCS6 device being enabled.  In cases where the ADCS6 unit was not
enabled, non-debug builds may crash on some host platforms depending on
compiler/memory layout.

dcd3e48048 AltairZ80: ADCS6: Initialize extended UDATA in reset routine.
As pointed out by @markpizz , the only allowed macro to initialize UNIT
structures is the UDATA macro.

Setting additional UNIT data structure items should be set through explicit
initialization code in the DEVICE reset routine.

5531ccb175 ALL: Massive 'const' cleanup
These changes facilitate more robust parameter type checking and helps
to identify unexpected coding errors.

Most simulators can now also be compiled with a C++ compiler without
warnings.

Additionally, these changes have also been configured to facilitate easier
backporting of simulator and device simulation modules to run under the
simh v3.9+ SCP framework.

9c4779c061 Notes For V3.8
The makefile now works for Linux and most Unix's. Howevr, for Solaris
and MacOS, you must first export the OSTYPE environment variable:

> export OSTYPE
> make

Otherwise, you will get build errors.

- BREAK, NOBREAK, and SHOW BREAK with no argument will set, clear, and
  show (respectively) a breakpoint at the current PC.

1.1.2 GRI

- Added support for the GRI-99 processor.

- Added support for the BACI terminal interface.
- Added support for RTE OS/VMA/EMA, SIGNAL, VIS firmware extensions.

- Added support for 12620A/12936A privileged interrupt fence.
